Family Case Manager

We are seeking a highly motivated and experienced Family Case Manager to join our team at Cafs. As a Family Case Manager, you will play a critical role in providing support and case management to families to promote the safety, stability, and development of vulnerable children and young people.
Key Responsibilities- Provide comprehensive case management services to families, including assessment, planning, and intervention.
- Develop and implement family action plans to address the needs and goals of families.
- Work collaboratively with multidisciplinary teams to provide a range of services that promote children's safety, stability, and healthy development.
- Authorise consultations with or reports to child protection when a child is believed to be in need of protection.
- Jointly respond with child and family services where child protection has an open case.
- Provide flexible packages to support a family action plan.
- Tertiary qualifications (Diploma and above) in Social Work, Welfare Studies, Family Therapy, Psychology, or related discipline.
- Proven experience in case management and working with vulnerable populations.
- Strong knowledge of the Best Practice Framework and ability to apply it in practice.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
