About the Role

We are seeking a highly skilled and compassionate Advanced Practitioner to join our team at Cafs. As a key member of our Strengthening Connections program, you will play a vital role in providing intensive case management and therapeutic intervention to young people and their families affected by adolescent family violence.

Key Responsibilities
  • Conduct comprehensive safety and wellbeing assessments to identify the needs of young people and their families
  • Develop and implement individualized care plans to address the root causes of family violence
  • Provide one-on-one and family counselling to promote healthy relationships and communication
  • Facilitate group work to strengthen parent-adolescent relationships and promote respectful communication
  • Collaborate with other professionals to ensure a holistic approach to supporting families
Requirements
  • Bachelor of Social Work or equivalent qualification
  • Experience in assessing risk and creating safety plans
  • Proven case management and therapeutic casework skills
  • Alignment to Cafs values of Respect, Integrity, Collaboration, Kindness & Innovation
  • Highly developed cultural awareness and sensitivity, with excellent communication skills
