We are seeking a highly skilled and dedicated Youth Services Case Manager to join our team at Cafs. As a key member of our team, you will be responsible for providing case management services to a range of young people, including high risk children and adolescents, who reside in a Cafs residential care unit or are supported by a Targeted Care Package.
Key Responsibilities- Provide case management services to young people in residential care and those supported by a Targeted Care Package
- Assist the team through the provision of specialised knowledge and guidance applied within the Youth Services programs
- Work collaboratively with stakeholders to achieve positive outcomes for young people
- Develop and implement individualised plans to support young people's needs and goals
- Monitor and evaluate the effectiveness of case management services and make recommendations for improvement
- Tertiary qualifications in Social Work, Welfare Studies, Family Therapy or equivalent
- Substantial experience in working with the Best Interests Case Practice Framework
- Highly developed cultural awareness and sensitivity, partnered with excellent communication skills
- Alignment to Cafs values of Respect, Integrity, Collaboration, Kindness & Innovation
