The Community Transitions Clinician plays a pivotal role in facilitating the transition of individuals with severe mental illness from custody to community care. This specialized clinician is responsible for collaborating with multidisciplinary teams across clinical, community, and custodial settings to ensure continuity of care and support.
About the Position
This dynamic role involves utilizing general care coordination and discipline-specific skills to link patients to relevant services in the community, thereby supporting their reintegration into community care.
Key Responsibilities
- Develop comprehensive mental health care plans that cater to the unique needs of patients leaving custody.
- Collaborate with multidisciplinary teams to establish therapeutic supports and ensure seamless care transitions.
Requirements
- Nursing staff: Possess current unrestricted Authority to Practice as a registered nurse with the Nursing and Midwifery Board of Australia, relevant post-registration qualifications in mental health, and at least 3 years' experience working in mental health settings.
- Allied health professionals: Hold a Bachelor or postgraduate degree in a relevant allied health profession (e.g., psychology, occupational therapy, or social work) and registration with the relevant professional board (Occupational Therapy Board of Australia / Psychology Board of Australia), or eligibility for membership with Australian Association of Social Workers; and at least 3 years' experience working in mental health settings.
