**Job Opportunity:**

We are seeking a dedicated and compassionate Care Coordinator Specialist to join our team.

This is an exciting opportunity for a motivated professional to make a real difference in the lives of children with complex health needs.

The successful candidate will be responsible for providing specialist care coordination services, ensuring that each child receives the highest level of care and support.

Key Responsibilities:

  • Providing direct care services to children, including outreach, assessment, care planning, and crisis intervention.
  • Developing and implementing individualized care programs tailored to each child's unique needs.
  • Addressing medical, social, developmental, behavioral, educational, and financial needs to promote healthy development.
  • Facilitating appointments, referrals, transportation, reminders, and follow-up with healthcare providers.
  • Maintaining accurate records and reporting to ensure timely and effective communication.
  • Liaising between management and disability support staff to ensure seamless collaboration.

Requirements:

  • Minimum 2 years experience working with children who have complex or additional health needs.
  • Knowledge of complex health needs in children and agencies engaged in assessing and providing services.
  • Excellent interpersonal skills and ability to communicate effectively verbally and in writing.
  • Confidence in dealing with other professionals and inter-agency discussions.
