FlexiHome Care has a very exciting opportunity for a Care Coordinator to join our expanding team
FlexiHome Care is a specialised provider of a range of disability support services, including quality in-home care support, accommodation support and life skills development. At FlexiHome Care, our goal is simple - we want our participants to be independent for as long as possible. We strive to get to know our participants and provide services that enable them to unleash their potential while making them feel comfortable with the support we provide.
We are looking for a fast-paced, enthusiastic and compassionate Care Coordinator to join and lead our Care Coordination team. This team is responsible for the coordination of resources and for managing our disability support care workers. Your role will be central in ensuring employees are working using safe practices, within guidelines and client's needs are being met.
**Your responsibilities will include**:
- Maintaining FlexiHome Care's service integrity and delivery quality.
- Supervising and managing all care staff including conducting spot checks, dealing with feedback, complaints and other issues.
- Overseeing rostering of support workers.
- Coordinating provision of quality person-centred care according to the client's needs, services/care plans and available funding in accordance with the budget.
- General administration with respect to tasks performed.
- Part take in ensuring WHS compliance.
- Processing new intakes and conducting care planning and reporting.
- Maintaining relationships with clients, carers, families, suppliers, community, industry and government.
- Training support workers on relevant duties/tasks.
- Updating care plans, risk assessments and SOPs on an ongoing basis.
- Leading the Care Coordination team, managing a maximum of 4 direct reports.

- Proficient with computers and Microsoft 365 suite.
- A proactive and problem solving orientated person.
**Essential Criteria**:
- Triple vaccinated (or willing to get their third dose)
- Experience in care coordination or registered nursing role
- Cert 3, 4 or Diploma in relevant field with 1 year experience as Care Coordinator or Registered Nurse OR
- Bachelor's degree in Nursing or related field, with little or no experience
- NDIS Worker Check
- Police Check
- Working With Children Check
- Right To Work - Citizen or Visa
**Job Types**: Full-time, Part-time, Casual
**Salary**: $28.00 - $34.00 per hour
Schedule:
- 8 hour shift
- Day shift
- Monday to Friday
- No weekends
Ability to commute/relocate:
- Bella Vista, NSW 2153: Reliably commute or planning to relocate before starting work (required)
