Care Coordinator – Full Time | 5 Days in Office

The RoleWe are seeking an experienced and dedicated Care Coordinator to join our Gregory Hills Head Office team on a full-time basis (Monday to Friday).
- Coordinate and oversee care plans, risk assessments, and service agreements.
- Supervise, mentor and support a team of Support Workers.
- Conduct client onboarding, home visits, and quarterly check-ins.
- Collaborate with clients, families, and stakeholders to ensure person-centered outcomes.
- Manage client documentation, incidents, complaints, and continuous improvement processes.
- Minimum 2 years' experience in care coordination or case management.
- Strong knowledge of NDIS, disabilities, complex care and mental health.
- Proven leadership and staff supervision skills.
- Excellent communication, organisation, and time management skills.
- Current Working with Children Check, NDIS Worker Screening, Police Check, Driver's Licence, and access to own vehicle.
- Relevant tertiary qualification in Disability, Allied Health, Social Science or similar (highly regarded).
