We are looking for support coordinators and senior support coordinators who value openness and good communication, who want to do their best and never stop being curious and open to learning. Perhaps you're an independent co-ordinator not wanting to go through the mandatory NDIS registration and looking to join an experienced team?
We are also looking for mental health clinicians (social work, psychology, occupational therapy, nurse) who have a strong passion for clinical excellence in the fields of Intellectual and Cognitive Disabilities.
In turn and in addition to competitive remuneration, COLLECTIVE provides a framework of clinical and operational support and potential for progression into senior role and partnership opportunities.
The Support Coordinator is to assist NDIS participants in the support coordination and implementation of all supports funded in their plan. The NDIA funds 3 levels of Support Coordination, including:
- Level 1 – Support Connection
- Level 2 – Coordination of Supports
- Level 3 – Specialist Support Coordination
Key Responsibilities:
- Provide the above levels of support coordination activities.
- Support the participant to implement their NDIS plan.
- Monitor plan outcomes and expenditure to ensure the plan remains in budget.
- Strengthen and enhance the participants' abilities to self-direct their own supports and participate in the community.
- Build effective relationships with participants and their families while maintaining appropriate and professional boundaries.
- Ensure mainstream services meet the obligations to the participant (housing, education, justice, health).
- Clear and accurate record keeping of information, time and billing.
- Build or support the capacity of teams to develop relationships and partnerships in local communities.

Skills and experience
- Allied Health qualifications or working towards will be preferred
- Knowledge and willingness to continue learning about NDIS operation, rules and regulation
- A self-starter, self-motivated person who can work independently
- Interest in working with people with a disability and mental illness
- Good knowledge of local disability, mainstream and community services
- Good verbal communication skills and a demonstrated ability to engage with clients and their families
- Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence and other documents
- Valid NSW Driver's Licence and access to own car
- Full Australian working rights required
- Reliable vehicle with comprehensive insurance for client visits required
