We are seeking experienced Nurse Practitioners to provide high-quality patient care in the comfort of patients' homes.

About the Position

The successful candidate will conduct comprehensive assessments of adult/geriatric patients, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to established guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.

Services will be provided in a variety of venues, including home visits and skilled facility visits.

Responsibilities
  • Conduct thorough Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to established guidelines and protocols.
  • Collaborate with healthcare professionals, including physicians, case managers, and social workers, to ensure comprehensive patient care.
  • Provide patient education and support, and collaborate with Primary Care Physicians on patient care plans.
  • Offer services in a variety of settings, including home visits and skilled facility visits.
Requirements
  • A Master's Degree in Nursing or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority.
  • Board certification by a recognized nursing organization, such as AANP or ANCC, in a relevant specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records software.
  • Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare teams.
  • Comfort and flexibility in adapting to changing circumstances.
