The Role

You will play a pivotal role by delivering person-centred care management to elderly clients in their own home. You will be an advocate, problem-solver, and partner in helping clients live fulfilling, independent lives.

Key Responsibilities:

  • Develop personalized care plans prioritizing independence, well-being, and dignity for clients.
  • Collaborate with internal teams ensuring seamless delivery of high-quality care.
  • Connect with clients, families, and stakeholders fostering trust, engagement, and exceptional outcomes.
  • Identify risk factors engaging in co-management approach to care planning.
  • Maintain accurate and timely records in client care management systems.

What We Look For:

  • A background in case management and aged care with a passion for making a difference.
  • Demonstrated experience in aged care as an Enrolled/Registered Nurse is highly regarded.
  • Able to thrive in a fast-paced environment, love problem-solving, and take initiative.
  • Excellent communication and interpersonal skills.
