About the Role

We are seeking a compassionate and skilled Pastoral Care Coordinator to join our team. As a Pastoral Care Coordinator, you will provide spiritual care and support to clients and their families in their own homes across Northern Sydney.

The role requires strong communication and interpersonal skills, with the ability to work effectively with clients, carers, and other stakeholders. You will also have experience in assessing client needs and developing individualized care plans.

  • Assessing client/family's spiritual and emotional needs and developing a spiritual care plan;
  • Providing spiritual and emotional support to clients and their families;
  • Liaising with religious/spiritual leaders to coordinate provision of spiritual support;
  • Contributing to carer support groups and other faith supporting activities;

To be successful in this role, you will require:

  • A commitment to HammondCare's Christian Mission, Motivation and Mission in Action;
  • Strong understanding and experience of people living at home with life-limiting illness including dementia, the impact on carers, and journeys of grief and bereavement;
  • Strong personal and active Christian faith and church involvement;
  • Theological /Pastoral Care qualifications and experience;
  • Demonstrated capacity to pastorally care for and enhance the wellbeing of those in need;
  • Highly developed communication skills including listening, compassion and empathy;
  • Experience raising up, leading and supporting a team;
  • Strong administrative and computer skills;
  • Access to a car, Australian's driver licence and comprehensive car insurance.
