Job Title: Disability Support Worker - Keilor East

We are seeking a highly motivated and experienced Disability Support Worker to join our team at Help at Hand Support Services in Keilor East. As a Disability Support Worker, you will play a vital role in providing person-centred support to individuals with disabilities, enabling them to live independently and participate fully in their communities.

Key Responsibilities:

  • Provide daily living support to individuals with disabilities, including personal care, meal preparation, and household management.
  • Assist with transportation and social activities, promoting social inclusion and community engagement.
  • Develop and implement individualised support plans, working collaboratively with clients, families, and other stakeholders.
  • Monitor and report on client progress, identifying areas for improvement and implementing changes as needed.

Requirements:

  • Relevant qualifications and experience in disability support or a related field.
  • Fluency in Sudanese language.
  • Current Working with Children Check and Police Check.
  • First Aid and CPR certification.
  • Full and clean Australian driver's license.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
