Job Title: Disability Case Manager

We are seeking a highly skilled and experienced Relationship Manager to join our team at our Bendigo office.

The successful candidate will be responsible for creating and maintaining relationships between participants and their team of support workers, assessing participant needs and developing Hearth's support program, and leading the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.

Key Responsibilities:
  • Creating and continuously developing relationships between participants and their team of support workers
  • Assessing participant needs and developing Hearth's support program
  • Maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders
  • Leading the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals
  • Reporting on the progress of clients as it relates to the support provided by Hearth and preparing submissions for NDIS plan reviews as required
  • Coordinating a weekly staff roster for each participant and proactively resolving rostering issues
Requirements:
  • Proven experience working in the disability sector preferably in a supervisory or managerial role
  • A Professional level of administrative and business communication skills including case noting and report writing
  • Excellent time management and organisational skills
  • Proven client-centred approach with the ability to advocate for participants and an understanding of complex disability and trauma
  • Own car and driver's license
  • Strong interpersonal skills and high levels of resilience
  • Understanding of the NDIS
  • Working knowledge of Microsoft Office suite and employee rostering systems
