About The Role
As central point of contact for support workers and participants, Relationship Manager role is core to what makes different to other support providers.
Relationship Managers ensure that participants, their family members, support workers, and all other stakeholders have a clear & consistent point of contact.

- Create and continuously develop relationships between participants and their team of Support Workers
- Assess participant needs, and develop Hearth's support program
- Maintain relationships with support coordinators, house supervisors, and other relevant external stakeholders
- Lead Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals
- Report on progress of clients, as it relates to the support provided by Hearth, and prepare submissions for NDIS plan reviews as required
- Coordinate weekly staff roster for each participant and proactively resolve rostering issues.
- Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan
- Compliance with policies and procedures to ensure a safe working environment and detailed, timely incident reporting
- Proven experience working in disability sector, preferably in supervisory or managerial role
- Professional level of administrative and business communication skills, including case noting and report writing
- Excellent time management and organisational skills
- Proven client centred approach with ability to advocate for Participants and an understanding of complex disability and trauma
- Own car and driver's license
- Strong interpersonal skills and high levels of resilience
- Understanding of the NDIS
- Working knowledge of Microsoft office suite and employee rostering systems
