
Disability Relationship Specialist

Hearth is a NDIS Registered Service Provider dedicated to creating an environment that nurtures, supports, and engages with our participants and support workers.
We foster and cultivate long-term and sustainable relationships through our committed team at our Bendigo office.
- Key Responsibilities:
- Bridge the gap between participants and their support teams by developing and maintaining meaningful relationships.
- Evaluate participant needs and develop tailored support programs that meet their unique requirements.
- Maintain strong connections with support coordinators, house supervisors, and external stakeholders to ensure seamless communication.
- Lead support worker teams in delivering person-centered care that empowers participants to achieve their goals.
- Monitor client progress and compile reports for NDIS plan reviews as required.
- Coordinate weekly staff rosters for each participant and address any rostering issues promptly.
- Accurately record case notes and conduct regular client reviews to inform forward planning aligned with the participant's NDIS plan.
- Ensure compliance with Hearth policies and procedures to maintain a safe working environment and adhere to timely incident reporting.
- Proven experience in the disability sector, preferably in supervisory or management roles.
- Demonstrated administrative and business communication skills, including case note taking and report writing.
- Excellent time management and organisational skills.
- A client-centred approach with the ability to advocate for Participants and understanding of complex disability and trauma.
- A valid driver's license and reliable transportation.
- Strong interpersonal skills and high levels of resilience.
- A working knowledge of the NDIS.
- Familiarity with Microsoft office suite and employee rostering systems.
