Support Client Independence

We are seeking a dedicated individual to join our team as a Supported Living Facilitator. In this role, you will work closely with clients living in their own homes, providing personalized support to enhance their quality of life and promote independence.

Key Responsibilities:
  • Provide comprehensive care services to clients, including personal care, daily living assistance, meal preparation, house cleaning, laundry, and ironing.
  • Maintain a safe and healthy environment for clients by driving between locations using your own vehicle.
  • Work mainly Monday to Friday, 9am-5pm, with occasional flexibility required.
  • Develop and implement individualized Care Plans to meet the unique needs of each client.
  • Encourage clients to participate in community-based recreational and social activities to foster connections and a sense of belonging.
  • Transport clients to appointments, shopping trips, and other essential errands.
Requirements:
  • Demonstrate strong interpersonal skills and ability to work effectively in a team-oriented environment.
  • Communicate clearly and concisely in spoken and written English.
  • Prior experience working in the Disability, Aged, or Community sector is advantageous but not mandatory; what matters most is a positive attitude and willingness to learn.
  • Hold a current NDIS Worker screening check.
  • Possess a valid CPR and First Aid Certificate (required before commencement).
  • Hold an Open Drivers License and have access to a reliable vehicle.
  • Car Insurance must be in your name.
