Empowering individuals with disabilities is at the heart of our organisation's mission. As an NDIS Support Coordinator, you will play a vital role in empowering participants to understand and implement their NDIS plans. This involves collaborating with them to interpret their plans, ensuring they comprehend their allocated funding and available support options.We are seeking a skilled professional who can provide personalised assistance to Aboriginal and Torres Strait Islander individuals with disabilities aged 0-64 in Southeast Queensland. The ideal candidate will have a relevant Tertiary qualification in disability support, social work, health, or a similar field, and/or possess 2 years' relevant experience in social services.The successful applicant will possess excellent communication, interpersonal, and advocacy skills, as well as the ability to network and develop effective working relationships. They will be able to identify and connect participants with suitable service providers and community resources, and develop individualised support plans aligned with participants' goals and aspirations.We offer a collaborative and person-centred approach that fosters independence and well-being for our clients.
