Comprehensive Case Manager

Our front-line support services involve delivering comprehensive individual counselling and case management to young people experiencing substance use and misuse, housing breakdown, mental health & relationship issues in Melbourne.
The role operates within a social model of health, conducting assessments, providing direct interventions, undertaking structured and semi-structured counselling sessions, and conducting supported referrals across domains.
We seek a passionate professional with at least 2 years of experience working in AOD and/or assertive outreach. You should hold a qualification in Social Work, Youth Work, Community Development, or similar disciplines and have completed (or be willing to complete) the AOD units of competency.
Key responsibilities include:
- Conducting thorough assessments and developing tailored intervention plans for clients
- Providing direct interventions, including brief advice, motivational interviewing, and solution-focused therapy
- Building strong relationships with local community service providers and other stakeholders to facilitate collaborative practice
Our team has helped over 15,000 individuals through face-to-face contacts and provided over 1.3 million safe injecting kits through our needle exchange program.
