Job Description

As a comprehensive case manager, you will play a vital role in delivering high-quality care services to clients across the Home Care Package program.

You will be responsible for developing and implementing personalized care plans, ensuring seamless coordination with internal and external service providers. Your goal will be to advocate for client needs, promote their independence, and enhance their overall well-being.

  • Client-Centered Approach: Work closely with clients, their families, and service providers to develop and document strengths-based care plans that meet individualized needs.
  • Care Coordination: Coordinate home care services, ensure timely delivery of support, and maintain open communication channels with all stakeholders.
  • Regular Reviews: Conduct regular review meetings to assess client needs, make necessary adjustments to care plans, and guarantee service delivery meets expectations.
  • Client Advocacy: Promote client rights and choices, empower them to make informed decisions about their care, and address any concerns or issues that arise.
  • Budget Management: Ensure accurate budget management, align services with available funding, and optimize resource utilization.
Required Skills and Qualifications

To succeed in this role, you will need:

  • Minimum Certificate III in Aged Care/Individual Support or equivalent experience in a similar field.
  • A genuine passion for supporting older adults and their families.
  • Strong understanding of Home Care Packages and the Aged Care system.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Effective time management and organizational skills.
  • Ability to work independently and collaboratively as part of a team.
  • Proficiency in MS Office Suite and client management systems (Etools/Carelink+ experience preferred).
