Case Management Professional

We are seeking a highly skilled and dedicated Case Management Professional to join our team. As a Case Manager - Reconnect, you will play a vital role in providing individual and family support to stabilise living situations and improve young people's connections with culture, family and community life.

The Uniting Reconnect Early Intervention Program is designed to help young people build resilience and achieve their full potential. You will use your community connections to undertake outreach and group work, facilitate referrals to specialist services, and advocate for participants, making a tangible impact on their journey toward stability and opportunity.

Your key responsibilities will include:

  • Stabilising living situations for young people and families through provision of tailored support and case management
  • Improving connections with culture, family and community life through community outreach and group work
  • Facilitating referrals to specialist services and advocating for participants

To be successful in this role, you will require:

  • A tertiary qualification in Social Work, Psychology, Welfare or related discipline
  • Demonstrated skills and experience in working with disempowered and vulnerable people
  • Ability to conduct strength-based assessments, develop intervention plans and conduct community outreach and group work
