As a Supported Living Facilitator, you will make a meaningful difference in the lives of individuals living in the community. You will provide care and support to clients, helping them maintain their independence and improve their quality of life.

Your Key Responsibilities
  • In-home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
  • Providing care in accordance with individualised Care Plans
  • Supporting with community-based recreational and social activities

You will spend your time on the road completing scheduled visits, interacting one-on-one with clients and their loved ones within their homes. This role requires flexibility and adaptability, as no two days are ever the same.
