Job Title: Community Care Specialist

We are seeking an experienced and qualified Community Care Specialist to provide person-centred care and support to clients with a variety of needs.


The successful candidate will have a Certificate III (or higher) in Aged Care, Home and Community Care, or Disability and at least three years' experience working in community or residential aged care.


Demonstrated knowledge and/or recent experience in aged care, including dementia care and manual handling, is highly regarded.


Key Responsibilities:
  • Developing, coordinating, and implementing care plans tailored to individual client goals.
  • Providing safe operation of hoists and other manual handling equipment.
  • Collaborating with families, carers, and multidisciplinary teams to ensure holistic care delivery.
  • Maintaining accurate, timely documentation in line with organisational and regulatory standards.
  • Promoting a safe, inclusive, and culturally respectful environment for clients and colleagues.
