Community Support Specialist
About the Role:The successful candidate will be responsible for delivering support services to individuals with diverse abilities. The role involves working closely with clients, families, and support teams to coordinate and implement personalized care plans.
Key Responsibilities:- Develop and implement tailored support plans that focus on enhancing independence, well-being, and social inclusion.
- Support clients with daily living activities, ensuring a respectful and dignified approach to care.
- Encourage clients to participate in community activities, social events, and recreational programs.
- Conduct regular assessments and interviews to identify client challenges and track their progress.
- Assess community needs and resources to design and implement programs addressing local health, housing, education, welfare, and employment issues.
- Provide education and care for children and individuals with disabilities in adult service units, group homes, and government institutions.
- Maintain detailed records of client interactions, progress notes, and any changes in care plans.
- Diploma, Advance diploma or Bachelor's degree in a related field.
- Minimum 2 years' experience working in community services or a similar field.
- Knowledge of relevant legislation and policies related to community services.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with clients, families, and multidisciplinary teams.
- Competitive salary: $75,000 - $80,000 per year.
