Senior Aged Care Professional

As a Senior Aged Care Professional, you will play a pivotal role in identifying the needs of older adults and connecting them with services that support their independent living.

  • Key Responsibilities:
  • Conduct thorough assessments to identify the needs of older adults, focusing on their functional capabilities and requirements.
  • Facilitate referrals to essential services, ensuring seamless coordination and support for individuals and their families.
  • Promote functionality by actively engaging stakeholders, including families, carers, and healthcare professionals, to maximize daily living abilities.
  • Ensure all assessments are conducted in line with Department of Health contractual requirements, maintaining the highest standards of compliance.

Requirements:

  • A minimum of 1 year of experience in aged care, including client assessments and service delivery.
  • Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office, including Word, Excel, and Outlook, as well as client management systems.
  • A valid Australian driver's license, comprehensive vehicle insurance, and ability to travel within designated service areas.
