Elderly Care Specialist

Job Summary:

We are seeking an Elderly Care Specialist to join our team. In this role, you will be responsible for conducting thorough assessments to identify the needs of older adults and facilitating referrals to essential services that support independent living.

Key Responsibilities:
  • Conduct comprehensive assessments to identify the needs of older adults and connect them with necessary services.
  • Promote functional independence in daily living by providing guidance and support.
  • Collaborate closely with families and caregivers to ensure seamless care coordination.
  • Ensure all assessments are conducted in accordance with relevant health department regulations.
Requirements:
  • At least 1 year of experience in elderly care, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted general registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and client management systems.
  • Current Australian driver's license and comprehensively insured vehicle for travel within your designated service area.
