Job Title: Social Care Specialist

A highly skilled and compassionate social care specialist is sought to provide professional level services in our Inpatient Rehabilitation Unit and Day Rehabilitation Unit.

The successful candidate will be responsible for delivering a comprehensive social work service, including psychosocial assessments, counseling, and discharge planning, in collaboration with our multidisciplinary teams.

  • To provide accurate and up-to-date records of patient interactions and interventions.
  • Contribute to quality improvement activities and participate in workplace education programs as required.

Key Requirements:
  • Qualifications in social work providing eligibility for membership of A.A.S.W.
  • Experience working with people from diverse cultural backgrounds.
  • Proven expertise in aged care, including psychosocial assessments, counseling, and discharge planning.
  • Strong advocacy skills and effective verbal and written communication.
  • Good knowledge of relevant community resources and experience liaising with community agencies.
