**About This Opportunity**

We are seeking a passionate and experienced Social Worker to join our dynamic multidisciplinary team on the 4D Renal Ward.

This ward plays a critical role in hospital patient flow and supports individuals with complex clinical and psycho-social needs, including co-morbidities such as substance use, homelessness, trauma, and mental health conditions.

**Key Responsibilities**

Provide specialist social work service to consumers and their carers to ensure optimal health outcomes consistent with policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

Participate in professional development, clinical supervision, teaching, quality activities and other departmental duties.

**Essential Skills and Qualifications**

- A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).

- Extensive postgraduate experience working as a Social Worker in relevant clinical areas, including understanding of health settings, complex health conditions and their impact on wellbeing.

- Experience working independently and collaboratively within a multi-disciplinary team.

- Demonstrated high-level communication skills (written and verbal), and computer skills.

- Expertise in provision of clinical supervision and consultation to social work services and clinical specialties.
