Job Title: Community Care Specialist

We are seeking a skilled and compassionate Community Care Specialist to join our team. As a Community Care Specialist, you will be responsible for delivering high-quality social work care to patients, families, and carers in a supportive healthcare environment.

  • You will work collaboratively with multidisciplinary teams to provide person-centered care, promoting the well-being and independence of our clients.

  • Your expertise in discharge planning, NDIS, child protection, domestic violence, grief, and bereavement will enable you to make a meaningful impact on the lives of our patients and their families.

Requirements:

  • Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).

  • Proven clinical experience in community-based settings, with a strong understanding of issues affecting Culturally and Linguistically Diverse client groups.

  • Excellent communication and interpersonal skills, with the ability to work effectively with people from diverse backgrounds.

  • Demonstrated ability to work independently and as part of a team, with a strong commitment to quality improvement and patient-centered care.

  • Proven experience in community-based practice, with a focus on enhancing patient outcomes and improving service delivery.

  • Strong understanding of current trends and best practices in social work, including evidence-based interventions and program development.

  • Ability to think critically and creatively, with a solutions-focused approach to complex challenges.
