
Social Care Specialist

We are seeking a highly motivated Family and Social Care Specialist to join our Women, Family and Emergency Care team at Royal North Shore Hospital. This role is an exciting opportunity for a dedicated individual to make a meaningful difference in the lives of women, children and their families.
The successful candidate will provide high-quality social work services to consumers, carers and families, ensuring optimal health outcomes consistent with NSW Health policies and procedures. They will work collaboratively within a multi-disciplinary team to deliver exceptional care and support.
To be successful in this role, you will possess a social work degree and eligibility for the Australian Association of Social Workers (AASW). You will also have proven ability to work independently and establish effective working relationships with internal and external stakeholders.

Key Responsibilities:
- Provide high-quality social work services to consumers, carers and families.
- Work collaboratively within a multi-disciplinary team to deliver exceptional care and support.
- Establish effective working relationships with internal and external stakeholders.
- Participate in professional development and quality improvement activities.
- Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
Requirements:
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
- Proven ability to work independently and establish effective working relationships within a multi-disciplinary team and interagency networks.
- Demonstrated well-developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
- Commitment to participation in professional development and quality improvement activities.
- Demonstrated computer literacy and ability to use health information systems.
