We are seeking an experienced and compassionate Community Care Registered Nurse to join our team.
The successful candidate will provide high-quality nursing care to clients in their own homes, working collaboratively with internal and external stakeholders to ensure the best possible outcomes.
- Lead and support the development of individualized care plans that cater to the unique needs and goals of each client.
- Provide expert guidance and mentorship to enrolled nurses and other healthcare professionals.
- Contribute to the development and implementation of policies and procedures that enhance the delivery of community-based care services.
Key Responsibilities:
Nursing Care:
Providing skilled and compassionate nursing care to clients in their own homes.
Assessing client needs and developing appropriate care plans to meet those needs.
Implementing and evaluating the effectiveness of care plans in collaboration with clients, families, and other healthcare professionals.
Leadership and Mentorship:
Providing leadership and guidance to enrolled nurses and other healthcare professionals.
Supporting the professional development of colleagues through coaching, mentoring, and training.
Policy Development and Implementation:
Contributing to the development and implementation of policies and procedures that enhance the delivery of community-based care services.
Participating in quality improvement initiatives to ensure continuous improvement of care services.
Requirements:
Education and Qualifications:
A Bachelor of Nursing degree from a recognized institution.
Experience:
Minimum 5 years of experience as a Registered Nurse, preferably in a community or aged care setting.
Skills and Attributes:
Excellent communication and interpersonal skills.
Strong clinical judgment and decision-making skills.
Ability to work independently and collaboratively as part of a multidisciplinary team.
Compassionate and empathetic approach to caring for clients and their families.
Working Conditions:
This is a full-time position, working 38 hours per week.
The successful candidate will be required to hold a current C-class driver's license and have access to reliable transportation.
