Chronic Health Nurse Specialist Role

We are seeking a highly skilled and dedicated professional to provide quality care and support to patients in regional Western Australia.

About the Position:

  • Conduct thorough health assessments, monitor disease progression, and respond to changing treatment needs.
  • Develop personalized care plans and collaborate with healthcare professionals to ensure seamless patient care.
  • Anticipate and address emerging health concerns of patients.
  • Deliver exceptional support to patients and families through prompt responses and feedback.
  • Maintain accurate and timely clinical documentation, including referrals.
  • Administer prescribed medications and treatments as per protocols.
  • Participate in or lead chronic disease management initiatives.

Essential Qualifications:

  • Experience in chronic disease management or postgraduate qualification in this field.
  • Bachelor of Nursing and current AHPRA registration.
  • Full working rights within Australia.
  • Willingness to relocate to a regional setting and adapt to new challenges.
  • Ability to pass a national police clearance check.
  • Strong organizational skills, capable of working under pressure, with excellent time management abilities.
