We are seeking a highly skilled and compassionate professional to fill the role of Chronic Health Registered Nurse in Perth, Australia.
Daily Responsibilities:- Conduct regular health assessments and monitor disease progression and treatment responses.
- Develop care plans and coordinate with healthcare professionals.
- Anticipate and manage changing health needs of patients.
- Support patients and families by responding to inquiries and feedback.
- Maintain accurate clinical documentation, including referrals, in a timely manner.
- Administer prescribed medications and treatments.
- Participate in or lead chronic disease management programs.
- Experience in the chronic disease field or postgraduate qualification in chronic disease.
- Bachelor of Nursing and AHPRA registration.
- Full working rights within Australia.
- Willingness to work in a regional setting and adapt to new environments.
- Ability to pass a national police clearance.
- Highly organized, capable of working under pressure, with strong time management skills.
