
Nurse Professional For Chronic Healthcare

As a Registered Nurse, you will have the opportunity to gain valuable experience while exploring the beauty of regional Western Australia.
Key Responsibilities:- Conduct regular health assessments and monitor patient outcomes.
- Develop and implement effective care plans in collaboration with healthcare professionals.
- Predict and manage changing health needs of patients.
- Provide exceptional support to patients and families through responsive communication.
- Maintain accurate clinical records, including referrals, in a timely manner.
- Administer prescribed medications and treatments as required.
- Participate in or lead chronic disease management initiatives.
- Experience in chronic disease management or postgraduate qualification in this field.
- Bachelor of Nursing and AHPRA registration.
- Full working rights within Australia.
- Ability to work in a regional setting and adapt to new environments.
- Pass a national police clearance.
- Proven organizational skills, able to work under pressure, with strong time management abilities.
