Chronic Disease Specialist Nurse

This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions.
- The successful applicant will provide clinical leadership in care coordination, overseeing comprehensive assessments and developing individual care plans.
- They will also support self-management through education and culturally appropriate health promotion.
- Collaboration with prison health services staff, general practitioners, mental health and allied health teams is essential for a patient-centred approach.
- Contribution to discharge planning and continuity of care, including liaison with community health providers and Aboriginal health services where relevant, is also expected.
This position plays a key role in policy development and implementation related to Chronic Disease Management.
Eligibility & Qualifications:- A Registered Nurse qualification is required for this role.
- The successful applicant must be an Australian citizen or have permanent resident status in Australia.
- A Working with Children Check is also required.
Travel to various sites may be necessary for this role.
