About the Role
The Department of Justice is seeking a dedicated and experienced Chronic Disease Nurse Coordinator to join the health services team within the Western Australian Corrective Services system.
This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key duties include clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans, and support for self-management through education and culturally appropriate health promotion.
You will collaborate with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach.
You will contribute to discharge planning and continuity of care, including liaising with community health providers and Aboriginal health services where relevant.
You will liaise with the local health department to place patients on appropriate registers to ensure continuity of care.
The role also forms part of the policy review team and will contribute to developing policies and procedures related to Chronic Disease Management.

Eligibility & Qualifications
Qualifications: the successful applicant must be eligible for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia.
Permanent positions: you must be an Australian citizen or have permanent resident status in Australia.
WWC positions: a current Working with Children Check or ability to apply for one is required for those working directly with children and young people.
Special conditions: a validCorC-Aclass driver's license or equivalent is required.
The position is based in Welshpool but travel to various sites may be required.

Evidence of initiative and problem-solving, including conceptual and analytical abilities.
A copy of your current AHPRA Nursing registration.
A copy of your current and validCorC-Aclass driver's license.
