The Western Australian Corrective Services system is seeking a dedicated and experienced Nurse Coordinator to join the health services team.
This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
About the Role- Clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans,
- Support for self-management through education and culturally appropriate health promotion,
- Liaising with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach,
- Contributing to discharge planning and continuity of care, including liaising with community health providers and Aboriginal health services where relevant,
- Liaising with the local health department to place patients on appropriate registers to ensure continuity of care.

Required Skills & QualificationsThe successful applicant must be eligible for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia. Permanent positions require applicants to be an Australian citizen or have permanent resident status in Australia. WWC positions require a current Working with Children Check or ability to apply for one.
