Job Title: Chronic Disease Nurse Coordinator

This is a rewarding opportunity to improve health outcomes for Western Australia's vulnerable populations. We are seeking a dedicated and experienced Chronic Disease Nurse Coordinator to join our health services team.


The successful applicant will be responsible for clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans, and support for self-management through education and culturally appropriate health promotion.


You will collaborate with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach. You will contribute to discharge planning and continuity of care, including liaison with community health providers and Aboriginal health services where relevant.


The role forms part of the policy review team and will contribute to developing policies and procedures related to Chronic Disease Management.


Required Skills and Qualifications:
  • Eligibility for registration as a Registered Nurse by the Nursing and Midwifery Board of Australia
  • Australian citizenship or permanent resident status
  • Current Working with Children Check or ability to apply for one
  • A valid C or C-A class driver's license
