About Our Opportunity

We are seeking an experienced and dedicated individual to join our team as a Care Coordinator/Nurse.

The successful candidate will preferably have a strong background in Community Aged/Disability Care and be responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support.

Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required (EN or RN).

This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.

Required Qualifications and Skills

A minimum of Diploma in Nursing with a current AHPRA registration;

Driver's License and comprehensively insured dependable private vehicle;

Current National Police Clearance; and

Current First Aid & CPR.
