Support Young People in Transition

We are seeking a Social Worker to join our team, working with young people aged 16-21 who are transitioning from care to independent living. Our goal is to provide them with the necessary skills and support to build a strong foundation for adulthood.

The Role

  • Build Trusting Relationships: Work one-on-one with young people to develop safe, respectful, and trusting relationships.">
  • Develop Everyday Life Skills: Help young people develop essential life skills such as budgeting, cooking, self-care, and routines.">
  • Support Identity and Belonging: Assist young people in strengthening their family, cultural, and community ties.">
  • Lead Case Planning: Collaborate with young people to set and achieve goals, while focusing on their strengths and challenges.">
  • Advocate for Needs: Ensure young people's voices are heard and their needs are met.">
  • Collaborate with Services: Work with child protection, health, education, housing, and other services to provide holistic support.">

Required Qualifications and Skills:

  • Degree in Social Work or Related Field: A degree in social work or a related field is required.">
  • Knowledge of Child Protection Responsibilities: Knowledge of child protection responsibilities and trauma-informed practice is essential.">
  • Experience Working with Young People: Experience working with young people with complex needs is necessary.">
  • Strong Case Management Skills: Strong case management skills are required.">
  • Ability to Build Genuine Relationships: The ability to build genuine, supportive relationships while maintaining professional boundaries is crucial.">
  • Driver's Licence: A driver's licence is necessary for the role.">
