We are seeking a passionate professional to join our team as a Home Transition Specialist. This role is part of our Housing Hub Team, dedicated to empowering individuals with disability to find their perfect home.

  • About the Role:
  • You will work closely with healthcare professionals, support specialists, and NDIS participants to identify housing needs and preferences, then conduct an innovative search for suitable options.
  • These options will be presented back to the professionals, including due diligence information, allowing the person with disability to choose the best fit for their needs and preferences.

Key Responsibilities:
  • Provide information and mentoring to key stakeholders to enhance housing-related searches for individuals.
  • Guide healthcare teams and other professionals through a housing needs and preferences conversation, promoting capacity-building in a person-led approach.
  • Conduct an innovative search for housing that aligns with the individual's needs and preferences.
Qualifications:
  • Hold an undergraduate allied health degree or possess extensive relevant experience working in the disability, health, housing, or social services sector.
  • Be eligible for relevant professional membership, providing evidence as required.
Desired Skills and Experience:
  • Experience working with people with disability and complex needs, particularly those in hospital or at risk of living inappropriately in aged care.
  • Commitment to honouring people with disability's rights, choice, control, and inclusion, especially in areas related to housing and support.
  • Facilitation of complex discussions with multiple stakeholders to reach an outcome.
  • Excellent working knowledge of NDIS, its framework, structure, and practice.
  • Demonstrated capacity to engage with healthcare professionals in a positive, meaningful way.
  • Confident building relationships within the sector.
