About the Role

We are seeking a dedicated and compassionate Enrolled Nurse to join our Home Care Services team in Darwin as a Home Care Coordinator.

Key Responsibilities:
  • Conduct onboarding, reviews, assessments, and develop budgets for clients in the region, ensuring their needs are met within their service budget and with a health-promoting focus.
  • Evaluate client services and set them up in a timely manner to ensure smooth delivery according to assessed needs, service plan, and budget.
  • Provide nursing services within the Scope of Practice and Competency Standards for Enrolled Nurses, prioritizing client well-being and safety.
  • Coordinate daily check-in meetings and facilitate communication among team members to address client concerns and escalate issues as needed.
  • Complete administrative tasks associated with the region, maintaining accurate records and reporting to support informed decision-making.
About You:

We seek an individual who embodies our values of Service, Courage, and Compassion. To be successful, you will have:

  • A Diploma of Nursing with current and unrestricted AHPRA registration as an Enrolled Nurse.
  • Current First Aid Certificate (HLTAID011) and CPR Certificate (HLTAID009).
  • A valid C-class driver's license.
  • Demonstrated experience working as an Enrolled Nurse in Community/Home Care or Residential Aged Care settings, along with experience in client support roles.
  • Strong interpersonal and communication skills, including proficiency in English language (written and verbal), to effectively engage with older adults and their families.
  • Effective time management and decision-making skills to autonomously manage your workload and prioritize tasks.
