Home Care Coordinator Enrolled Nurse Role

At Southern Cross Care, we are seeking a dedicated and compassionate Enrolled Nurse to join our Home Care Services team in Darwin.
The successful candidate will be responsible for providing person-centered services to clients, conducting onboardings, reviews, assessments, and developing budgets for clients in the region.
Key responsibilities include ensuring client services are set up in a timely manner, providing nursing services in line with the Scope of Practice and Competency Standards for Enrolled Nurses, coordinating and leading daily check-in meetings, and completing administrative tasks associated with the region.

","requirements":"To be successful in this role, you will require:
- Diploma of Nursing with current and unrestricted AHPRA registration as an Enrolled Nurse.
- Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009).
- Current and unrestricted driver's (C class) licence.
- Demonstrated experience working as an Enrolled Nurse in a Community / Home Care or Residential Aged Care setting, as well as experience in client support roles.
- Sound interpersonal and communication skills, including a high level of proficiency in the English language (both written and verbal), with demonstrated ability to communicate effectively with older people with complex needs.
- Time management skills to autonomously manage own workload.
- Effective decision making ability, including the ability to identify potential risks and escalate as appropriate within the organisation.
- Proficient computer and keyboard skills, including the use of community aged care systems/processes (eg, MyAged Care, ACAT, RAS etc).
