About The Position

The Rehabilitation Coordinator forms part of the Offender Services team, responsible for developing and delivering programs and services across specialist portfolios: transition and reintegration, family engagement, disability, diversity, and complex care.

The coordinator administers the Reintegration Pathway, including risk and needs assessments, transition planning, parole application processes, and referrals to pre and post-release services.

Duties and Responsibilities:
  • Support delivery of rehabilitation programs in a prison environment.
  • Administer risk and needs assessment tools to identify rehabilitation requirements and make referrals as needed.
  • Work with multi-disciplinary teams to develop integrated transition plans, actively involving prisoners in goal setting and program participation.

  • Effective stakeholder management skills.
