Job Opportunity

The role of Care Coordinator is a vital position that acts as the primary liaison between clients, their families, and service providers.

This position involves overseeing the implementation of clients' plans to ensure services are delivered effectively and in alignment with individual goals.

Key Responsibilities:

  • Manage client needs and goals through assessments and care planning.
  • Ensure excellence in client service delivery, including goal planning and onboarding of new clients.

Requirements:

  • Current driver's license and a vehicle with third-party property insurance.
