**About the Role**:
As a Care Coordinator, you will serve as the primary point of contact for participants, their families, support workers, and various stakeholders. Your role is essential in ensuring seamless coordination of support services, particularly in behavioural support, mental health, and complex care needs. You will manage participant care, collaborate with allied health professionals, and ensure adherence to NDIS requirements.

**Key Responsibilities**:

- **Behavioural Support & Mental Health**: Provide coordination and support for participants with behavioural and mental health needs.
- **Participant Relationships**: Develop and nurture strong relationships between participants and their team of Auslands Care Support Workers.
- **Allied Health Coordination**: Collaborate with allied health professionals to ensure participants receive the right interventions and care.
- **HR and Staff Management**: Oversee recruitment, training, and performance of support workers in line with NDIS requirements.
- **NDIS Plan Reviews**: Report on the progress of participants and prepare detailed submissions for NDIS plan reviews.
- **Roster Coordination**: Coordinate weekly staff rosters and proactively resolve any rostering issues.
- **Daily Reporting**: Write detailed daily reports, including case notes and client reviews, ensuring compliance with NDIS goals and participant needs.
- **Compliance & Safety**: Ensure all policies and procedures are followed, including incident reporting and maintaining a safe working environment.

**Key Requirements**:

- Minimum 2 years experience working as a Care Coordinator, Case Manager, Support Coordinator, or similar role in disability services or community care.
- **Experience in Behavioural Support and Mental Health** is essential.
- Strong understanding of the NDIS framework and the role of Disability Support Workers.
- Experience liaising with **allied health professionals** to deliver comprehensive care.
- **HR experience** in managing staff, recruitment, and training.
- Strong administrative, organisational, and time management skills.
- **Proficiency in writing daily and weekly case notes, as well as detailed submissions for NDIS plan reviews..**:

- Strong interpersonal skills and high levels of resilience.
- Working knowledge of Microsoft Office suite and rostering systems.
- Own car and valid driver's license.
