Role Summary

This Aboriginal Health Pathways Clinician position offers a unique opportunity to provide culturally responsive services to clients on the Out-of-Home-Care (OOHC) Health Pathway Program.

Key Responsibilities

  • Provide strength-based, trauma-informed, and person- and family-centred services to clients.
  • Develop and maintain connections with Aboriginal Community Controlled Organisations and community services.
  • Collaborate with multidisciplinary healthcare teams to deliver quality assessment, review, and care coordination and navigation services.

Requirements

  • Qualifications in Social Work, Psychology, Occupational Therapy, or Counselling.
  • Demonstrated core competency skills in culturally informed assessment and care coordination and navigation within an Out of Home Care or trauma setting.
  • Excellent written, verbal, and interpersonal communication skills.
  • Demonstrated understanding of challenges faced by Aboriginal children and young people in accessing healthcare services.
  • Commitment to clinical and cultural supervision and professional education.

Work Environment

This role will be part of the South Eastern Sydney Local Health District, providing community health services and population health services to the community.
