Aboriginal Health Navigator

This role provides comprehensive support to children, young people and their families, facilitating access to health services and enhancing overall well-being outcomes.

Main Responsibilities:
  • Deliver culturally sensitive and person-centred services to clients on the OOHC Health Pathway Program, focusing on strengths-based approaches and trauma-informed care.
  • Establish and maintain relationships with Aboriginal Community Controlled Organisations, community services and local communities, promoting collaboration and coordination.
  • Work effectively within a multidisciplinary team to deliver high-quality assessment, review and care coordination and navigation services.
  • Demonstrate exceptional written, verbal and interpersonal skills to engage clients and foster effective teamwork.
Essential Qualifications and Skills:
  • Possession of qualifications in Social Work, Psychology or Occupational Therapy, accompanied by relevant registration.
  • Proven core competency in culturally informed assessment and care coordination and navigation within Out of Home Care or trauma settings.
  • Demonstrated understanding of challenges faced by Aboriginal children and young people in accessing healthcare services.
