About the RoleThis is an ongoing full-time or part-time position, with a minimum of 4 days per week. You will be working with a diverse range of participants, their families, and support teams to develop new skills and habits, and to overcome barriers that prevent them from engaging with their local community and the wider world.
Key Responsibilities- Work alongside participants and their key supports to develop new skills and habits, and to problem-solve challenges that prevent them from living fulfilled lives.
- Champion the role of a social model practitioner, helping people to overcome barriers and engage with their community.
- Use evidence-based practice and professional reasoning to recommend the most appropriate team of supports, therapy, duration, location, and timing.
- Develop effective and strong working relationships with people and their families, identifying and addressing factors that may affect relationships, and repairing breakdowns in a timely manner.
- A competitive salary of $83,582 - $100,342, including annual leave loading and superannuation.
