Key Responsibilities:

  • Conduct initial assessments using our LEAD evidence-based assessment tool
  • Liaise with the interdisciplinary team to provide wrap-around support
  • Support participants in their everyday environments
  • Capacity build support teams to provide capable environments
  • Participate in peer group supervision, mentoring, and professional learning
