
Advanced Clinical Practitioner

This role requires a skilled professional to provide clinical case management for patients with long-term conditions.
Main Responsibilities:- Maximise independence by supporting people with complex needs to remain in their own homes as appropriate.
- Undertake clinical assessments, provide treatment and link with existing services to facilitate early discharge from hospital and prevent re-admission.
- Conduct comprehensive health and social care assessments, utilising current information to develop individualised plans of care for patients.
- Review medical history, physical examination, medication review and prescribing in conjunction with management plans.
- Make referrals for diagnostic tests and functional/cognitive assessments.
- Manage complex interventions for individuals within an identified patient group on an ongoing basis.
- Undertake risk assessment and monitor changing clinical and social situations.
The ideal candidate will have extensive post-registration experience, management/clinical leadership experience and a relevant clinical qualification. They must also possess excellent interpersonal skills, IT proficiency and the ability to work under pressure.
